About this service

Eligibility criteria

For adults who are registered with a GP in the NHS Mid Essex area and housebound. 

Service Details

We provide a nursing service for those who, for reasons of acute or chronic illness, are unable to leave their home by domestic vehicle.

We can offer the following treatments:

  • Wound management – for trauma and chronic wounds.
  • Lower-leg care – venous, mixed, diabetic or arterial ulceration requiring assessment, dressing or monitoring.
  • Post-operative care – Once ambulant you will be referred on to your GP Practice nurse. 
  • Administration of medication inclusing IV therapy. (Excluding oral medications and long term eye drops)
  • Treatment and monitoring of health problems.  Long-term support of chronic degenerative conditions (such as MS, paraplegia, dementia, CCF).
  • Preventing health complications – includes flu vaccinations.
  • Management of urinary catheters and bowel management.
  • Terminal or palliative care.
  • Enabling self care to patients and their carers – includes administering injections, PICC lines, eye drops and dressings.
  • Avoiding unnecessary hospital admission through patient and carer education, symptom management, enrolment on to various healthcare programmes, adoption of telehealth programmes to empower patients to self manage their chronic conditions.

We also have an End-of-Life Clinical Facilitator, who can help patients, and their families, who have a life-limiting illness and are in the last stages of life.

Our team includes qualified specialist practitioners, nurses and healthcare assistants.

Service Delivery

We only provide care to house-bound patients.

The service is available between 8am and 11pm, Monday to Sunday and on Bank Holidays. Outside of these hours please contact you GP's out of hours provider.


Referral process

You must be referred to our service by a healthcare professional, such as your GP or Acute Hopsital Trust.

  • 100% of people said our Community Nursing staff were professional, courteous, presentable and pleasant.
  • 100% said the Community Nurses treated them with respect and dignity.
  • 100% said the nurse clearly explained their treatment and any equipment they might need.
  • 100% said they were involved as much as they wanted to be in decisions about their care and treatment.
  • 100% said any important questions were answered clearly.
  • 97% said they knew who to contact if they were concerned with any aspect of their care.
