About This Service

Eligibility Criteria

People over 18 years, living and registered with a GP within Mid Essex.

Service Details

The service aims to prevent and avoid admissions to hospital by using its contacts and networks in the community.  It also works closely with hospitals to enable patients to be safely discharged early from hospital and resettled back home. 

The service runs seven days a week between 8am and 11pm

Service Delivery

We have nurses responding to urgent calls within the community covering all of Mid Essex.

We have nurses working in the acute hospital within the emergency department and within the wards.

We have a Central Point Of Access in Chelmsford which houses a Triage Nurse, as well as telephonists within a call centre who provide onward referrals and signposting services

Referral Process

Referrals can be received from anyone as long as they are over 18 and are registered to a Mid Essex GP.
