Eligibility criteria

The ESDAAR Service is for adults aged over 18 years, living and registered with a GP within mid-Essex.

Service Details

A team of nurse assessors that deliver a rapid triage and deployment model through a Care Co-ordination Centre hub facility. Offering skilled assessment and treatment of patients and complete input by proving a comprehensive handover to the most appropriate health, social or voluntary sector agencies for the patients’ needs in a holistic manner.

The service runs seven days a week from 8am to 11pm.

Service Delivery

We have nurses responding to urgent calls within the community covering all of mid-Essex.

We have nurses working in the acute hospital within the emergency department and within the wards.

We have a Care Co-ordination Centre which houses a Triage Nurse, as well as Care Coordinators who provide onward referrals and signposting services.

Referral Process

Referrals can be received from anyone as long as they are over 18 and are registered to a mid-Essex GP.

If you are registered with SystmOnline you will be able to view your own Electronic Medical Record (EMR), manage your appointments, request repeat medication, update contact details and message your healthcare service. There is also a SystmOnline app available for Apple and Android devices.
