Provides self management plans for people diagnosed with Angina and long term medical management for those with heart failure, from diagnosis to end stage.

About this service

Eligibility Criteria

Adults who are registered with a GP in the NHS Mid Essex area and who meet the criteria as set down by the CCG. Your healthcare professional will be able to advise you on the criteria needed

 

Service details

We treat patients with a diagnosis of Angina and on medical treatment. Offering "The Angina Plan", a 12-week self management plan that includes support and encouragement from the service. Exercises can be done at home or at the cardiac rehabilitation class which the service runs.

We provide long-term medical management and support to patients with heart failure, from diagnosis through to end-stage of the disease.

The service ensures that patients with heart failure are taking appropriate medication to control any heart failure symptoms, and that they have the most appropriate treatment for their heart failure.

We offer:

  • Clinic and home visits – to assess and educate, and optimise medication.
  • Inpatient hospital visits – to assess prior to discharge.
  • SOS response – patients can contact the service and arrange a visit for an assessment to to help prevent the need to attend hospital
  • Cardiac rehabilitation – an eight-week course of exercise, based on ability. We'll provide the patient with information about their condition and how best to manage it.

We offer these treatments:

  • Clinical assessment, including blood pressure and pulse oxygen saturation.
  • ECG (heart monitoring), including 24-hour ambulatory monitoring.
  • 24-hour ambulatory blood-pressure monitoring.
  • Venepuncture (blood taking).
  • Optimising medication, appropriate to the condition.
  • Cardiac Rehabilitation classes

We treat patients who are referred to the Cardiology GPwSI, completing an assessment and provision of a management plan to the patients' GP.

Our team includes specialist nurses, healthcare assistants and Cardiology GPwSI.

Service delivery

We run clinics and rehabilitation classes at various times Monday to Friday, 9am-5pm, at these venues:

  • Broomfield Hospital, Chelmsford.
  • Halstead Hospital Rehabilitation and Podiatry Unit,
  • St Michael's Health Centre, Braintree,
  • St Peter's Hospital, Maldon
  • Burnham Clinic, Burnham-on-Crouch

* Appointments must be booked. Any related issues please contact the Cardiac Service and NOT the venue.

We also make home and hospital visits where necessary.p

Referral process

Ask your GP or health professional to refer you to our service.

If you are a patient already known to us, you can call the service after being discharged for information and advice, using the details in the contacts box on this page.
