The Care Co-ordination Centre (CCC) operates from 8am to 6pm Monday to Friday*. If you need to make contact with any of our services within the CCC, contact the team today.

*Integrated Care is open 8am to 11pm Monday to Sunday.

What is the Care Co-ordination Centre?

The Care Co-ordination Centre (CCC) provides a single access point into the organisation for all customers. The CCC team responds to customers’ needs and deals with enquiries, prioritising urgent need within a seamless integrated system. The CCC team manages the needs of customers by maintaining standardised processes across all services. This includes:

  • Managing the booking, cancellation and re-scheduling of appointments for Provide services
  • Managing referrals into Provide services
  • Ensuring that the ‘Referral to Treatment’ procedure is adhered to
  • Ensuring a seamless flow of information between Provide services and customers
  • Providing advice and signposting customers to other appropriate services that may help them

Integrated working with mental health services and social care services to reduce the number of contacts required to be made by our customers.

The Care Co-ordination Centre will be the go to place where all customers know that their enquiries and needs will be dealt with sensitively and professionally, treating customers with care and compassion.

If you are registered with SystmOnline you will be able to view your own Electronic Medical Record (EMR), manage your appointments, request repeat medication, update contact details and message your healthcare service. There is also a SystmOnline app available for Apple and Android devices.
