Eligibility criteria

Adults (18+) registered with a GP in the NHS Mid Essex area.

Exclusions

Exclusions include those who:

  • can access the Outpatient MSK Physiotherapy Service.
  • require provision of long-term equipment (over six months) or major adaptations only. They should be referred to the social care occupational therapy team.
  • are already under the care of hospice therapists.
  • have recently been discharged from our service and whose condition has not altered.

Service details

Our physiotherapists and occupational therapists offer assessment and rehabilitation to people who have a health-related condition which affects their ability to walk and move around or to manage everyday activities, such as preparing meals, washing and dressing.

We work with patients to agree rehabilitation goals to improve independance and safety within everyday activities:

  • Home exercise programmes.
  • Practising or adapting everyday activities.
  • Advice and education.
  • Provision of walking aids, other equipment and minor adaptations.

Service Delivery

The service operates Monday to Friday, 8.30am-4.30pm, delivering care in people's homes or as inpatients on the wards.

We are based at three hospitals:

  • Braintree Community Hospital
  • Halstead Hospital
  • St Peter’s Hospital.

Referral process

You must be referred by a health or social care professional, such as your GP or social worker.

You will be seen within 18 weeks of referral. If you have an urgent problem which would result in a hospital admission without our help, we will see you within two working days.
