Falls Prevention Service

Providing assessment, advice and support (including exercise classes) for people who have fallen or are at a risk of falling, to help prevent future falls.

About This Service

Eligibility criteria

This service is for adults registered with a GP in the NHS Mid Essex area who:

  • have had a fragility fracture - hip, wrist, spine.
  • have had a fall, slip or trip in the past 12 months.
  • have gait or balance problems.
  • have been assessed by a clinician as at high risk of falling.
  • are on medication known to cause falls, such as psychotropes, antiarrhythmics, antihypertensives, opiates or sedatives.
  • have a long-term condition, such as Parkinson’s, stroke, or arthritis.

Exclusions

  • Those receiving care in an acute hospital environment for an injury/trauma, or an episode/exacerbation of illness or self-harm.
  • Those receiving a period of intensive support to enable them to remain in their own home. For example, if they're not mobilising or transferring on their own or with assistance and/or are being hoisted.
  • Those affected by cognitive impairment or significant confusion, but only where there is established evidence that engagement with the programme would offer no benefit and every attempt has been made to address their needs.

Service details

We provide free interventions for people who have fallen or have a fear of falling and are losing confidence in moving about safely.

Based on an individual's needs, a care plan will be prescribed. This may include:

  • Home hazard assessment.
  • General health and wellbeing advice.
  • Free three-month trial of Carecall support (when they have not previously had a carecall alarm).
  • Falls exercise classes.
  • Nurse and/or therapy assessments and treatment plans.
  • Home exercises.
  • Regular follow-ups to monitor progress.
  • Referral to specialist services.

 

Service Delivery

Our service operates 9am-5pm, Monday to Friday at variety of locations across the locality.

Nurse-led clinics take place 9.30am-3pm at the following venues:

  • Mondays: Springfield Clinic in Chelmsford
  • Tuesdays: Alternated between Burnham and South Woodham Clinics
  • Thursdays: Halstead Hospital 

Falls classes take place at the following venues:

  • Tuesday: Landers Court, Chelmsford, 1-3pm
  • Wednesday: St Peter's Hospital, Maldon, 10am-12 noon
  • Thursday: South Woodham, 1.30-3.30pm; Halstead Hospital, 10.30am-12.30pm, Great Baddow, 12.24-2.45pm and Witham 10am-12 noon
  • Friday: Braintree 12.30-2.30pm and Burnham 12.30-2.30pm

  • You can self refer by phoning or emailing using the contact numbers on this page.
  • You can also be referred by a social or health care professional.
