About This Service

Eligibility criteria

Children under five with identified or emerging disabilities registered with a GP in the NHS Mid Essex area.

Service details

We work with children up to five years of age (and their families) who have identified or emerging disabilities. We will visit a child at home, pre-school, school or other community setting.

We also deliver multi-agency training for health, education, social care and other agencies around the Early Support Approach and the ten principles which underpin it.

Our practical resources and training enable a way of working which:

  • Helps practitioners and managers to put the child and their family at the centre of holistic, coordinated and seamless service delivery.
  • Supports children and their families to take control of their lives, gain a firm basis of knowledge and communicate their expertise in a way that enables them to make informed choices, take the lead in decision making and become active partners in service planning, improvement and delivery.
  • Provides the context in which services can work in an integrated way with a single assessment and planning process.

We work with health, education, social care and Voluntary organisations to coordinate and organise the "Team around the Child" approach.

An early support admin coordinator liaises and coordinates with the family and professionals to ensure the key professionals are able to attend a "Team around the Child" meeting, at a time and venue suitable for the family. They will also type and circulate the family service plan after the meeting

Parents are given the opportunity to hold a "Team around the Child" meeting at home or a health setting, children's centre, local nursery, pre-school or school. These are held around every four to six months, depending on the level of need.

"Team around the Child" attendees can include healthcare professionals, such as paedatricians, GPs, therapists, nurses and health visitors, as well as educational psychologists, social workers and education staff.

Service Delivery

We run our service Monday to Friday, 9am-5pm, at:

  • Moulsham Grange Children's Centre in Chelmsford.

We also visit children at home, school or other community settings.

Referral process

  • You child can be referred by a healthcare professional, such as your GP or health visitor.
  • Or you can self-refer by calling our Early Support Link Practitioner on the number in the contacts box on this page.
