About This Service

Eligibility criteria

Anyone up to 16 years old (or 19 for those with special needs) with type 1 diabetes, living in the NHS Mid Essex area.

Service details 

We work closely with paediatricians and staff on the children’s ward at Chelmsford's Broomfield Hospital to provide support and on-going care to children and young people with type 1 diabetes, and their families.

Our team includes both specialist nurses and dieticians, working together to provide children and young people and their families with appropriate education and information to allow them to manage their condition effectively.

Specific adolescent diabetes service provision needs to be targeted, as their needs are different and they require more psychological input.

Children and young people are seen in the multidisciplinary team clinics every three months.

We aim to:

  • Minimise the length of stay in hospital following diagnosis, by supporting discharge home.
  • Be flexible in response to the child or young person and their family’s needs.
  • Work with the Health Improvement team to develop health promotion information for children.

Service Delivery

We run multidisciplinary team clinics every three months. Clinic visits are arranged in between home visits.

Home visits continue until needs have been established and are on-going.

Referral process

You must be referred by a healthcare professional, such as a GP, acute consultant or ward staff.

We will contact you to arrange your first visit within two days of referral, excluding bank holidays and weekends.

A plan of care will be discussed with the child or young person and their family.
