About This Service

Eligibility criteria

Telecare service

Available to anyone living in the NHS Mid Essex area who considers that they would benefit from an alarm.

Telehealth service

Available to patients with a long-term condition, such as COPD and/or heart failure. Patients must be under the care of  COPD, Heart Failure or Integrated Care teams, and be referred by them. Patients must have sufficient manual dexterity and capacity to use the device and associated peripherals (such as BP monitor) and have had two recent hospital admissions or serious exacerbations in the past year.

Service details

We offer two services:

Telecare

A 24-hour alarm monitoring service. The person wearing the alarm, or the device itself, can call for help from anywhere in their home and get straight through to the control centre, which is open 24 hours.

The alarm location and address is automatically detected in seconds. Once connected, the alarm holder is able to talk to trained control centre staff and, if needed, help can be on its way in minutes.  All calls are recorded and the service is provided in line with Telecare Services Association code of practice which Carecall is accredited to.

Telehealth

Provision of telehealth systems to enable patients with a long-term condition to monitor their health within their own home.

Results of SATs and health-related questions are downloaded on a daily basis to clinicians within Provide’s long-term condition teams, including COPD, Heart Failure teams, GPs and matrons in the Integrated Care Team (ICT). This enables clinicians to detect early signs of exacerbation of condition and provide prompt intervention to avoid acute exacerbation and unnecessary hospital admission.

Service Delivery

The Carecall office is based in Braintree and open Monday to Friday, 8am-6pm.

Referrals can only be made by Provide's COPD, Heart Failure or Integrated Care teams.
