Medicines Management is a system of processes and behaviours that determines the way that medicines are used by patients in the NHS.

We continuously strive to ensure the safe use of medicines within our services, and have developed the Pharmaceutical Care Plan (see document below) for our inpatient units to support medicine use and ensure that our patients understand their medicines.
We have published information leaflets to help our patients manage their medicines.
This is complemented by signposting patients to community pharmacies where their services may be beneficial to patients.

The Medicines Management Team audits and evaluates the use of medicines within our services by undertaking audits with feedback reports, actions, progress and compliance monitoring.
The audits review the safe and appropriate use of medicines and ensure that this is in line with current best practice guidance. Where necessary, actions are taken to identify areas of improvement which are monitored and reported to local teams, the Medicines Management Committee, Quality and Safety Committee and the Board.
We produce quarterly and annual reports which reflect on our practice and identify key themes for learning and improvement.

We support the development of our workforce via training to sustain and improve skill mix, knowledge and processes relating to medicines management across the organisation.
The Board monitors medicines management through the bi-monthly Quality and Safety Board Report.
The Medicines Management Team is actively involved in ensuring the safe use of medicines across the organisation and this is monitored via the Medicines Management Committee, Medication Safety Group and the Quality and Safety Committee.
